You may have seen the images or videos. A person appears to be standing or sitting, but their upper body is dramatically bent forward — sometimes almost at a right angle. Their head hangs down, their arms may dangle toward the ground, and they can appear almost frozen in that position.


The phenomenon has become known online as the “fentanyl fold.”


But what actually causes it?

And why can someone remain in such an unusual position without apparently correcting themselves?


The answer involves one of the most dangerous effects of powerful opioids: the suppression of the brain's control over consciousness, movement and breathing.



Fentanyl doesn't simply make someone “high”

Fentanyl is a synthetic opioid used medically for severe pain and during some surgical procedures. Pharmaceutical fentanyl is extremely potent — roughly 50 to 100 times more potent than morphine.

Illegally manufactured fentanyl is a major driver of opioid overdose deaths, and it can be present in powders or counterfeit tablets without the person taking the drug knowing exactly what they have consumed.


At sufficient doses, opioids suppress activity in the central nervous system.

The person becomes increasingly sedated. Their awareness decreases. Their muscles may relax or, paradoxically, some muscles can become extremely rigid.


And, most importantly, their breathing can become dangerously slow or stop altogether. That combination can produce some remarkably abnormal-looking postures.

So why do they fold forward?

There isn't one single explanation for every person displaying this posture.

A person experiencing opioid intoxication or overdose can have severely impaired consciousness and motor control. They may lose the normal ability to maintain an upright posture while still having enough muscle tone to remain standing.


Gravity then does the rest.


The upper body begins to collapse forward while the legs continue supporting some of the person's weight.

Instead of simply falling to the ground, they can become suspended in an extreme forward-bent position.


Recent medical literature has specifically described this forward-flexed posture as the “fentanyl fold,” particularly in connection with fentanyl and fentanyl-xylazine exposure.

However, it is important not to assume that every person in this position has taken fentanyl. Similar postures can potentially occur for other medical or drug-related reasons.



There is another strange effect: rigidity

Fentanyl has another property that can make opioid overdoses particularly dangerous. In some circumstances, fentanyl can cause severe muscle rigidity involving the chest, jaw, neck and other muscles.

Doctors call this phenomenon fentanyl-induced chest wall rigidity, or sometimes “wooden chest syndrome.”


Instead of becoming completely floppy, a person can become unusually stiff.

Research has found that fentanyl can produce rapid rigidity in the muscles involved in breathing, potentially making it extremely difficult for the chest to expand normally.


This helps explain why fentanyl overdoses can sometimes look very different from the stereotypical image of someone simply collapsing unconscious.


One person might become limp. Another might become rigid. Another might appear frozen in an abnormal posture.

And some people may exhibit combinations of these effects.



The person may not realise what is happening

This is perhaps the most important part of understanding the phenomenon. Someone experiencing severe opioid intoxication isn't necessarily consciously deciding to remain in that position.

As opioid effects increase, awareness and responsiveness can deteriorate. The brain's ability to respond normally to discomfort, imbalance and changes in posture can become severely impaired.


Normally, if you begin leaning forward too far, your nervous system automatically corrects the movement.

You straighten your back. You move your feet. You sit down. You catch yourself. But when consciousness and motor control are severely impaired, those automatic corrections can become ineffective.


The result can be an extraordinarily unnatural-looking posture.



The danger isn't the posture — it's the breathing

The folded position itself isn't necessarily what kills someone. The major danger is respiratory depression.

As an opioid overdose progresses, breathing can become slow, shallow or irregular. Eventually, breathing can stop.


The resulting lack of oxygen can cause brain injury, cardiac arrest and death.

That means a person who appears to be merely “nodding off” or standing strangely may actually be experiencing a medical emergency.


This is one reason health authorities warn that it can be difficult to distinguish between someone who is simply intoxicated and someone experiencing an overdose.



Why fentanyl can make the situation particularly dangerous

Fentanyl acts rapidly and is exceptionally potent. Illicit fentanyl can also be mixed with other substances.

That creates another layer of uncertainty: the person taking a drug may not know exactly what is in it.


The CDC reports that illegally manufactured fentanyl is commonly found in powders and counterfeit pills and can be mixed with substances such as heroin, cocaine and methamphetamine.


More recently, fentanyl mixed with xylazine — sometimes called “tranq” — has received increasing attention.

Xylazine isn't an opioid, so naloxone does not reverse its effects. However, naloxone should still be administered when an opioid overdose is suspected because fentanyl may be involved.



What should you do if you see someone like this?

Don't assume they're simply sleeping or intoxicated.

If someone is difficult or impossible to wake, has slow or shallow breathing, makes choking or gurgling sounds, or has discoloured lips or skin, treat it as a potential opioid overdose.


If naloxone is available, administer it and call emergency services.

Naloxone can rapidly block the effects of opioids and restore breathing. The CDC notes that more than one dose may sometimes be required when fentanyl is involved.


Stay with the person and follow emergency medical advice.



The image can be misleading

Perhaps the most important lesson from the “fentanyl fold” is that what an overdose looks like isn't always what people expect an overdose to look like.

It doesn't necessarily involve someone dramatically collapsing. Someone can still be standing. They can still appear to be breathing. They can even appear to be partially conscious.

Yet their brain and respiratory system may already be dangerously compromised.


The strange folded posture is therefore more than an unsettling image circulating on social media.


It is a visible reminder of what can happen when a powerful opioid interferes with the brain's control over consciousness, movement and breathing.

And recognising the signs could mean the difference between someone simply appearing intoxicated and someone receiving potentially lifesaving treatment in time.
